Licensed Vocational Nurse (LVN)

The licensed vocational nurse is responsible for providing direct patient care under the supervision of a registered nurse. Responsibilities include following the plan of care, providing treatments, and working collaboratively with the members of the team to help meet positive patient care outcomes. Duties:

• Provide primary care to specific residents under the medical direction and supervision of the residents’ attending physicians or the Medical Director of the facility, with an emphasis on assessment, illness prevention and health care management.
• Assist in modifying the treatment regimen to meet the physical and psychosocial needs of the resident.
• Administer medications as directed by resident’s physician.

Requirements:

• Must possess a current, unencumbered, active license to practice as an LVN in California.
• Current CPR certification.
• Must pass pre-employment back ground check and physical.
• Point Click Care experience preferred.
• Provide direct patient care, related to end-of-life care
• Implement current nursing practices following plan of care
• Provide accurate and timely documentation of patient services
• Asses and provide patient and family/caregiver education
• Participated in coordination of hospice services
• Uses and prepares equipment and supplies effectively
• Provides appropriate pain/symptom management
• Participates in personal, professional growth and development
• Perform other duties assigned by RN
• Answer multiple-phone calls
• Screen and triage patient calls, and take messages.
• Coordinate services between staff and patient.
• Completing projects or special assignments when requested.
• Copy and assemble reports or documents as needed.
• Order medical equipment and supplies as requested
• Obtaining insurance authorization
